Hybrid Care Manager

📍Crown Heights
4-6 weeks in-person training in Crown Heights and then remote after that
☀ 9-5 availability, Mon-Fri

Oversee 25 patients who are receiving a slew of services, including a full-time home aid, home-delivered meals, social daycare, home visits by medical personnel, transportation to and from appointments, medical supplies, and more.

As a care manager you will coordinate the moving pieces so that everything runs smoothly for the patients and their family members.

Position Overview: 
- check in with patients over the phone 
- ⁠de-escalate when patients are dissatisfied 
- ⁠complete paperwork by deadlines 
- ⁠conduct 8 home visits monthly around 5 boroughs - MUST BE COMFORTABLE GOING INTO PATIENTS HOMES
- ⁠BA or Associates required!
