📍Crown Heights: 4-6 weeks full time, in-person training and then remote
☀️ 9-5 availability, Mon-Fri
As a case manager you will coordinate the moving pieces so that everything runs smoothly for your 25 patients and their family members, helping them navigate challenges as they arise.
Position Overview:
- check in with patients over the phone
- de-escalate and problem solve when patients issues arise
- complete state paperwork accurately and ensure all deadlines are met
- conduct 8 home visits monthly around 5 boroughs - MUST BE COMFORTABLE GOING INTO PATIENTS HOMES
- BA required!
