Service Coordination Supervisor

In this role, you’ll guide a coordination team, support day-to-day operations, and help create a smooth, thoughtful experience for both patients and staff.

What you’ll do:

• Lead and support a team of 5–10 Service Coordinators
• ⁠Run focused 1:1s and team meetings
• ⁠Oversee patient outreach, documentation, and retention
• ⁠Step in on escalations and improve workflows
