Case Manager (Hybrid)

🏡 Crown Heights | 4–6 weeks in-person training, then hybrid 9–5 (Mon–Fri)

As a Case Manager, you’ll be the steady point person for 25 patients and their families coordinating care, resolving issues as they arise, and helping everything run smoothly.

Position Overview:

• Check in with patients regularly by phone
• ⁠De-escalate concerns and problem-solve in real time
• ⁠Complete state paperwork accurately and meet all deadlines
• ⁠Conduct 8 home visits per month across the 5 boroughs (must be comfortable entering patients’ homes)
• ⁠Associate’s or Bachelor’s degree required
